Qualified Social Worker | Hospital Team | Weston-Super-Mare

  • Qualified Social Worker required to join a statutory Hospital Team
  • Office base: Town Hall, Walliscote Grove Road
  • To be considered, you must have a social work qualification and full SWE registration as well as over 3 years post qualified experience gained in an Adults Services setting

  • As a result of hospital winter pressures we are looking for 2 social workers with hospital discharge experience to join North Somerset Council's Hospital Discharge Assessment Team. The timeframe is for an initial three months.

    The team is in the Adult Social Care directorate, with a remit to support adults ready for discharge from hospital who have care and support needs. Staff work mainly from home with access to the office at Weston-super-Mare Town Hall when necessary for support, supervision and team meetings.

    Our assessments are undertaken at the end of a D2A discharge pathway. The majority of the work is undertaken in the community in the person's home or in care home or community rehab placements, with some inreach assessments in our acute hospitals.

    A friendly and professional team, HDAT has been fortunate to maintain a committed, experienced staff group. HDAT pride themselves on their proactive, whole team approach for decisions and developments and actively support staff development in learning and career progression. As a proactive, responsive service, the team has a good working relationship with the community health provider and the local acute trusts.

    The work is fast paced and requires a social worker capable of quick turnover, short-term interventions.

    Applicants should have excellent organisational and communication skills and an ability to self-motivate, work autonomously and manage their time effectively. Good record keeping skills are critical in ensuring the team are updated on case progression through shared case notes (LAS).

    Supervision is provided by an allocated Senior Social Worker.

    The team consists of the Team Manager, Senior Social Workers, Social Workers and Adult Social Care workers. The team follows up discharges with a review in the community following the initial assessment.

    Due to North Somerset having many rural areas not served by public transport, we require a car driver for this post.
