Key Responsibilities:

  • Conduct assessments to determine individual needs and develop personalised care plans.
  • Provide support, advice, and interventions to individuals, carers, and families to promote independence and well-being.
  • Work within the Care Act 2014, Mental Capacity Act, and safeguarding procedures to ensure legal compliance and best practice.
  • Collaborate with health professionals, housing services, voluntary organisations, and other key stakeholders to provide holistic support.
  • Monitor and review care plans to ensure they remain effective and responsive to changing needs.
  • Manage a caseload, prioritising tasks effectively to meet deadlines and service objectives.
  • Advocate on behalf of service users, ensuring their voices are heard in decision-making processes.
  • Keep accurate and up-to-date case records and reports in line with professional and legal requirements.
  • Participate in team meetings, supervision, and continuous professional development activities.

    • Essential Requirements:

  • Degree or equivalent qualification in Social Work.
  • Social Work England registration (or eligibility to register).
  • Experience in adult social care, mental health, or disability services.
  • Strong knowledge of relevant legislation, including the Care Act 2014 and safeguarding policies.
  • Excellent communication, assessment, and problem-solving skills.
  • Ability to work effectively both independently and as part of a multidisciplinary team.
  • A commitment to anti-discriminatory practice and promoting equality and diversity.
  • Proficiency in using case management systems and IT software.
  • Full UK driving licence and access to a vehicle (if required for the role).
