About the Role:

We are seeking a dedicated and compassionate Social Worker to join our Adults Team. The successful candidate will be responsible for assessing, supporting, and safeguarding vulnerable adults, ensuring they receive the necessary care and services to promote independence, well-being, and quality of life. You will work collaboratively with individuals, families, and multi-disciplinary teams to develop and implement effective care plans.

Key Responsibilities:

  • Conduct comprehensive needs assessments, risk assessments, and care planning in line with legislative frameworks (e.g., Care Act 2014, Mental Capacity Act 2005, and safeguarding protocols).
  • Provide professional social work support to adults with physical disabilities, learning disabilities, mental health needs, or age-related conditions.
  • Work collaboratively with health professionals, carers, voluntary organisations, and community resources to coordinate appropriate care and support.
  • Advocate for service users, ensuring their voice is heard and their rights are upheld.
  • Maintain accurate and up-to-date case records using appropriate systems.
  • Participate in safeguarding investigations and ensure the protection of vulnerable adults.
  • Support service users in accessing appropriate housing, employment, and community integration services.
  • Keep up to date with policy changes, best practices, and developments in adult social care.
  • Requirements:

  • Degree or postgraduate qualification in Social Work.
  • Registration with Social Work England.
  • Proven experience in adult social care, ideally within a statutory setting.
  • Strong knowledge of relevant legislation, including the Care Act 2014, Mental Capacity Act 2005, and the Equality Act 2010.
  • Excellent communication, problem-solving, and decision-making skills.
  • Ability to work independently as well as part of a multidisciplinary team.
  • Proficiency in case management and report writing.
  • A valid UK driving licence (preferred but not essential, depending on the role)
