Job Overview:

We are seeking a dedicated and compassionate Adults Social Worker to join our Learning Disabilities (LD) Team. The successful candidate will be responsible for providing high-quality social work support to adults with learning disabilities, promoting their independence, well-being, and social inclusion. This role requires strong assessment, advocacy, and partnership-working skills to ensure individuals receive the necessary care and support tailored to their needs.

Key Responsibilities:

  • Conduct strength-based assessments and develop person-centered care plans in line with the Care Act 2014.
  • Work collaboratively with healthcare professionals, families, and partner agencies to ensure coordinated support.
  • Support individuals with transition planning, ensuring continuity of care and support when moving between services.
  • Conduct Mental Capacity Assessments and implement Best Interests Decision-making processes where applicable.
  • Provide safeguarding interventions to protect vulnerable adults from abuse or neglect.
  • Ensure compliance with legislative frameworks, including the Mental Health Act 1983 (amended 2007), Mental Capacity Act 2005, and the Care Act 2014.
  • Maintain accurate and timely case records and reports using relevant IT systems.
  • Support service users in accessing community resources, housing, employment, and educational opportunities.
  • Contribute to service development, innovation, and quality improvement initiatives.
  • Essential Criteria:

  • Qualified Social Worker with a Degree or Master's in Social Work.
  • Registration with Social Work England.
  • Experience working with adults with learning disabilities in a social care setting.
  • Strong knowledge of legislation, policies, and best practices in adult social work.
  • Experience in carrying out risk assessments, safeguarding investigations, and care planning.
  • Ability to work effectively within a multi-disciplinary team.
  • Excellent communication, advocacy, and interpersonal skills.
  • A proactive and compassionate approach to empowering service users.
  • Competence in IT systems and case management software.
  • A valid UK driving license (desirable)
