About the Role

We are seeking a dedicated and experienced Hospital Social Worker to join our dynamic team within the [Name of Hospital or NHS Trust]. This role is vital in supporting patients and their families during hospital stays, ensuring safe and effective discharge planning, and promoting patient independence.

Key Responsibilities

  • Undertake assessments of patients' social care needs, ensuring appropriate care plans are in place.
  • Work closely with multidisciplinary teams, including doctors, nurses, and therapists, to facilitate effective discharge planning.
  • Provide emotional support and advocacy to patients and their families during challenging times.
  • Arrange and coordinate care packages, liaising with local authorities, community services, and other stakeholders.
  • Ensure compliance with the Care Act 2014, Mental Capacity Act, and other relevant legislation.
  • Conduct safeguarding investigations where necessary, ensuring the protection of vulnerable individuals.
  • Maintain accurate and timely records in line with professional and legal standards.
  • Promote independence and well-being by signposting patients to relevant support services.
  • Requirements

  • Qualification: Degree in Social Work (or equivalent).
  • Registration: Social Work England registration is essential.
  • Experience: Previous experience in a hospital or healthcare setting is highly desirable.
  • Knowledge: Strong understanding of UK social care legislation, safeguarding procedures, and hospital discharge processes.
  • Skills: Excellent communication, problem-solving, and interpersonal skills.
  • Flexibility: Ability to work under pressure in a fast-paced environment and adapt to changing circumstances.
  • Full time post - Driving is essential

    This is a G7 SW role carrying out D2A discharges from Pathway1, Pathway 2 and Pathway 3 settings and completing safeguarding enquiries.

    We would expect the person to be available Monday to Saturday in Wolverhampton as discharge and safeguarding scenarios can require urgent unplanned visits in the community. We therefore would need the person to be able to attend urgent visits as and when needed.

    Our G7 staff cover Saturday duty. This will be 1 Saturday in 5 working from home, taking the following Monday off. The expectation is they are able to attend any urgent visits either at New Cross Hospital or in the community should need arise while covering Saturday duty.

    Should we require the person support the Duty team, we would expect them to be able to be office based 3 days a week. They will need to be available to attend urgent duty visits either in the hospital or in the community when they are working from home.
