We are recruiting 3 experienced social workers with recent experience of working within statutory Hospital Adult Care teams to undertake complex hospital discharges for Rochdale residents and a key role is under taking Mental Capacity Assessments for residents within a Discharge to Assess placements. An excellent understanding of the Mental Capacity Act and an ability complete mental capacity assessments for specific decisions on Long Term Residency and Finances is an essential requirement.
You will be expected to undertake statutory Social Work functions in line with the Care Act and the Mental Capacity Act. To undertake duties expected of Social Workers in line with the Professional Capabilities Framework and in accordance with the specific job description attached.

Our Hospital Discharge Flow Hub supports Rochdale residents with discharge arrangements, timely and safely to a suitable setting. We utilise various Discharge to Assess and re-ablement pathways to support individuals to maximise their independence away from the hospital setting, with a focus on discharge to the right place at the right time. The team is responsible for working with people to assess both the immediate and ongoing needs, in order to support a person in implementing their longer term care and support needs. The team is located in the Middleton area to support discharge planning across the various hospital sites in and out of the borough as part of a multi-agency team to facilitate effective, safe and timely discharges.

Minimum of 2 years hospital social work experience.
You will have an excellent understanding of the Mental Capacity Act and practical experience of completing specific MCA's for Long Term residency and Finances
You will be able to work in a fast paced environment which is an essential requirement
You will be an effective multi-tasker with excellent organisational skills and the ability to work flexibly to meet the needs of the service.
You will be motivated and forward thinking, who can demonstrate professionalism and enthusiasm
You will be a team player
You will have excellent interpersonal and communication skills
You will have excellent ICT skills and be able to use a range of packages including Word, Excel and Outlook.
You will have a real attention for detail and be able to maintain accurate and detailed records using both manual and electronic systems. A knowledge of liquid logic is beneficial
