Key Responsibilities
✅ Needs Assessments: Conduct strength-based assessments under the Care Act 2014 to determine eligibility for support. ✅ Care Planning: Develop, implement, and review person-centred care and support plans that promote independence. ✅ Safeguarding: Identify and manage risks, ensuring safeguarding procedures are followed in line with The Care Act 2014 and Making Safeguarding Personal principles. ✅ Multi-Agency Collaboration: Work closely with health professionals, housing teams, voluntary organizations, and other key partners. ✅ Direct Support & Advocacy: Empower individuals by ensuring their voices are heard and advocating for their rights. ✅ Mental Capacity & Best Interests: Conduct Mental Capacity Act (MCA) 2005 assessments and contribute to Best Interests decision-making processes. ✅ Commissioning Services: Arrange and monitor support services such as home care, supported living, and direct payments. ✅ Carer Support: Assess and support unpaid carers, ensuring their well-being and involvement in care planning. ✅ Record Keeping: Maintain accurate and up-to-date case records in line with legal and organisational requirements.
