We are seeking a dedicated and experienced Adult Social Worker to join our Community Learning Disabilities Transition Team. This role is vital in supporting young people with learning disabilities as they transition from children's services into adult social care, ensuring a seamless and person-centred approach to independence, well-being, and social inclusion.

To coordinate the work of the Children Services and Adult Services in the transition of young people with social care needs as they enter adulthood.
The role will ensure that high quality and timely transition support is offered to young people and their carers in accordance with Care Act duties and the principals of Preparing for Adulthood.
The role has a specific focus on working with colleagues and external partners to develop effective transition arrangements for young people with the most significant needs.
The role will support engagement with Children and Young People's Health, Social Care and SEND services to support effective pathways for young people to develop their independence.
The role will support effective planning and procurement of local supported living services for young people with complex needs, offering high quality and cost-effective alternatives to out of area placements.
The role will work in collaboration with other Team Manager colleagues in the Learning Disability Team and Children with Disabilities Team as part of a combined management team.
Deputise for the Service Manager as required and keep them apprised of sensitive, high-profile issues.
Responsible for quality assurance and contribute to the strategic direction of the service.
