Job Overview:
We are seeking a dedicated and experienced Qualified Social Worker to join our Adults Mental Health Team. You will be responsible for delivering high-quality, person-centred support to adults experiencing mental health challenges, ensuring statutory responsibilities are met in line with the Care Act 2014, Mental Health Act 1983, and other relevant legislation.

Qualified Social Worker with evidence of relevant qualifications Minimum of 5 years post qualifying experience Registered with Social Work England Experience of working in an Adult Social Care statutory setting Knowledge and experience of working with people who have severe and enduring mental ill-health Experience of undertaking assessments of need, undertaking scheduled and unscheduled reviews, creating care-plans and commissioning support to meet identified needs. Experience of statutory safeguarding under s42(1) and s42(2) Care Act Experience of completing Mental Capacity Assessments Knowledge and understanding of the Care Act, Mental Capacity Act, Mental Health Act, Equalities Act, Human Rights Act, Data protection and GDPR, Continuing Health Care. Knowledge and understanding of Deprivation of Liberty Safeguards. 2 x references Gaps in employment history accounted for Full clean driving license

Due to career progressions, Agency Social Worker opportunities are now available within the the Mental Health Review Team in Middleton and CMHT West in Rochdale. The role is to undertake social work statutory functions within an Adult Care mental health service. We are co-located with health colleagues and work in a multi-disciplinary way.
We require agency social workers to undertake Strength Based Needs assessments, review packages of care, update care plans, undertake mental capacity assessments where appropriate, undertake safeguarding work where appropriate and facilitate hospital discharges for people who have a severe and enduring mental health diagnosis and who are under secondary mental health services. Individuals may live within the borough of Rochdale or may be placed out of borough. It will be helpful to have an understanding of mental health pathways.
WORKING PATTERN:
37 hours per week Monday to Friday There is some scope to work from home with manager agreement, however the role is primarily office based due to the needs of the client group
