Job Overview:We are seeking a dedicated and compassionate Adults Social Worker to join our dynamic team in Wirral, UK. The successful candidate will work with vulnerable adults, supporting them to live independently, safeguarding their rights, and promoting their well-being. You will be responsible for conducting assessments, developing care plans, and working collaboratively with multidisciplinary teams to ensure the best outcomes for service users.Key Responsibilities:Conduct strength-based assessments of adults in need, including older adults, those with disabilities, and individuals experiencing mental health challenges.Develop and implement personalised care plans, ensuring they align with statutory guidelines and best practices.Support individuals and their families to make informed decisions regarding their care and support needs.Work closely with other professionals, including healthcare providers, occupational therapists, housing officers, and community support services.Ensure compliance with relevant legislation, including the Care Act 2014, Mental Capacity Act 2005, and Safeguarding Adults framework.Conduct safeguarding investigations, identify risks, and implement protection plans where necessary.Maintain accurate and up-to-date case records in line with local authority policies and statutory requirements.Provide crisis intervention and respond to urgent safeguarding concerns.Promote independence, choice, and control for service users while ensuring their needs are met effectively.Attend court proceedings and prepare relevant report writing as required.Essential Criteria:Degree in Social Work (BA or MA) or equivalent qualification.Registered with Social Work England.Previous experience working with adults in a social work setting.Strong knowledge of legislation, policies, and best practices in adult social care.Ability to conduct holistic assessments and develop person-centred support plans.Excellent communication, interpersonal, and report-writing skills.Ability to work autonomously as well as part of a multi-disciplinary team.Experience in handling safeguarding cases and risk assessments.Strong problem-solving and crisis intervention skills.A full UK driving licence and access to a vehicle (desirable but not essential).
