We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's Social Work Hospital Team in Bournemouth. This role is pivotal in providing high-quality support to adults with complex needs, ensuring their well-being and safeguarding their rights. As part of a multidisciplinary team, you will work collaboratively with healthcare professionals to deliver effective assessments and interventions for individuals in hospital settings.

Your key responsibilities will include:

Conducting comprehensive assessments of adults' social care needs within the hospital environment. Developing and implementing person-centered care plans in collaboration with patients, families, and healthcare teams. Providing emotional and practical support to patients and their families during challenging times. Ensuring compliance with relevant legislation and safeguarding policies. Maintaining accurate and timely documentation of case notes and reports.

To be considered for this role, you must possess the following essential qualifications and skills:

A valid Social Work qualification (BA/MA in Social Work or equivalent). Registered with Social Work England. Proven experience in adult social work, particularly within hospital settings. Strong communication and interpersonal skills, with the ability to work effectively in a team. Excellent assessment and analytical skills, with a focus on person-centered approaches.
